Rental information (Distributor / Contact / Type of Equiptem(s) / Rental Rate
The Lessee as named above, hereby agrees to rent the described equipment (the Equipment) from Simtech Industrial Products., as
indicated below (the Lessor), commencing the shipment date, in accordance with the following schedule and the attached Terms
and Conditions.
The appropriate rental rate will be charged from the time the Fusion Equipment is received on silte and will conclude when the
Fusion Equipment is receives at Simtech's facilities (the Rental Period). All rentals are on a calendar day basis. For monthly
rental amount apply daily rate times 15 ( Per day X 15). All freight charges for the Equipment are the responsibility of the
Lessee.
The Equipment will be delivered in good condition and will be accompanied by an itemized list of parts and attachments. Upon
return of all equipment to Simtech, a thorough inspection will be made. ALL REPAIRS REQUIRED TO RESTORE THE
EQUIPMENT TO ITS PRE-DELIVERY CONDITION (EXCEPT FOR NORMAL WEAR AND TEAR) WILL BE CHARGED
TO THE LESSEE AT $50.00 PER HOUR FOR LABOR PLUS PARTS. IF THE EQUIPMENT IS NOT RETURNED OR IS
RETURNED IN UNUSABLE CONDITION, LESSOR WILL BE RESPONSIBLE FOR ALL RENTAL CHARGES PLUS
STANDARD PURCHASE PRICE FOR THE EQUIPMENT.
Leased equipment will be invoiced upon shipment to the Lessee at the full purchase price along with applicable freight charges.
Applicable rental charges will be billed on a monthly basis. Upon receipt of the equipment by the Lessor from the Lessee, the
Lessor will calculate any applicable rental charges in accordance with above rates and determine any cost of repair. Lessor will
then issue credit equal to the invoiced price less rental charges, cost of repair and freight.
By singing and below the Lessee hereby agrees to the rates, terms and conditions as described above and on the attached Terms
and Conditions (page two of rental agreement)
